AllThrive 365 to Continue Duet Programs, Expanding Support for Homebound Adults, Family Caregivers, and Grandfamilies
Transition ensures continued access to vital aging services for Arizona families
Phoenix, AZ. (August 4, 2026) – Beginning August 4, 2026, Phoenix-based nonprofit, AllThrive 365, will continue three long-standing programs previously offered by Duet: Partners In Health & Aging, ensuring that homebound adults, family caregivers, and grandfamilies across Arizona continue to receive essential support and services.
Founded in 1981 by The Reverend Dr. Dosia Carlson and the Church of the Beatitudes in Phoenix, Duet has spent more than four decades providing free-of-charge services that promote health, independence, and connection for older adults and families. Following Duet's decision to sunset operations, its Homebound Adult Services, Family Caregiver Services, and Grandparents Raising Grandchildren Program will continue through AllThrive 365 beginning August 4, 2026.
The transition reflects a shared commitment to ensuring these services remain accessible and sustainable for the thousands of Arizonans who rely on them each year.
"AllThrive 365 is honored to carry forward the impactful programs that Duet has built over more than four decades," said Tami Bohannon, President and CEO of AllThrive 365. "These services reflect the very heart of what we believe—that no one should have to navigate aging, caregiving, or life transitions alone. As we continue this work, we are committed to honoring the relationships, compassion, and trust that have always defined it, while expanding access to health, housing, and connection for even more of our Arizonan neighbors."
Together, AllThrive 365 and Duet represent a combined 97 years of service to Arizona communities. The addition of these programs strengthens AllThrive 365's ability to support individuals and families through some of life's most complex and meaningful transitions.
The programs include:
•Homebound Adult Services, connecting older adults with volunteers who provide companionship, transportation, light handyman services, paperwork and tech support, and more.
•Family Caregiver Services, offering education, support groups, resources, and guidance for family caregivers.
•Grandparents Raising Grandchildren, supporting grandparents and relatives who have stepped into parenting roles and need emotional, educational, and practical resources.
These programs served approximately 3,000 Arizonans in 2025.
"This program transfer is the strongest path to preserving the core services, relationships, and values that have shaped Duet's mission for more than 45 years," said Cheryl Plewa, Board President of Duet. "We are deeply grateful to AllThrive 365 for their shared commitment and care for the continuation of services to homebound adults, family caregivers, and grandfamilies."
For AllThrive 365, the transition represents a natural extension of its mission to champion health, housing, and connection for aging Arizonans. The organization serves tens of thousands of individuals statewide through programs focused on affordable housing, healthy aging, caregiver support, transportation, home and community-based services, and resource navigation. Funds to explore this transition were generously provided by Arizona Together for Impact.
